In 2371 the U.S.S. Voyager was violently swept into the Delta Quadrant, the entire medical team was killed and the EMH also called The Doctor was activated. The Doctor was only a temporary solution as it was built for emergencies and only for short term use but this is not the case as he is the only source of medical treatment aboard Voyager.

The EMH was designed by Dr. Lewis Zimmerman at the Starfleet's Jupiter Station. He had designed it to operate for no more than 1500 hours. Because Voyager had the EMH as their only medical officer that limit was exceeded and the EMH suffered a level-4 memory fragmentation, resulting in rapid deterioration of program function. Instead of reinitiating the EMH the Voyager crew decided to merge the EMH's program matrix with the Jupiter Station's EMH diagnostic program. After Stardate 50312.5 when Voyager went back into the past to 1996, he obtained a 29th century technology, and autonomous holo-emitter which allowed him to operate outside the sickbay.

The Doctor was programmed with over five million possible treatments, he was programmed with information from 2000 medical references and the experience of 47 physicians. He had the knowledge three thousand cultures. Even though The Doctor is a holographic object he is considered to be a part of the crew as he now has been active from the beginning when Captain Janeway lost her medical team during the displacement of her ship by Caretaker technology.
